大ヒットゲーム「バイオハザード」のフル3G長編アニメ第2弾! 旧ソビエトの社会体制崩壊後、独立し主権国家となった東スラブ共和国。しかしその後、富裕層(オルガルヒ)の支援を受ける政府側と、貧困層の民衆で構成される反政府勢力側との間で紛争が繰り返されていた。この内乱が過激化する中、各地で「戦場でモンスターを見た」という奇妙な噂が流れ始めるのだった。この情報を受けた米国はB.O.W.拡散を危惧し、大統領直属エージェント、レオン・S・ケネディを現地に単独潜入させる。だが、政府事情により米国は突如撤退を決め、レオンにも退去命令が下る。「B.O.W.の手掛かりがここにある以上、俺は残る―。」独り戦場へ駆け出すレオンだったが、リッカー(B.O.W.)が現れ、反政府勢力側に拉致されてしまう。反政府勢力のメンバーであるアレクサンドル(サーシャ)・コザチェンコ/バディ(相棒)とJD、長老会議の指導者アタマン(長老)であるイワン・ジュダノビッチ、そして椅子に縛られたレオン・・・。
