3代将軍・家光の時代から幕末・大政奉還にいたるまで、男女が逆転した江戸パラレルワールドを描いた、よしながふみの傑作コミック『大奥』。同作を原作に、8代将軍・吉宗までの物語を描き好評だった、ドラマ10『大奥』の新章がスタート! 吉宗の遺志を継いだ蘭方医たちが、男性だけに感染する謎の疫病"赤面疱瘡 (あかづらほうそう)"の撲滅に立ち向かう姿とその後の物語を描く『医療編』、女将軍をはじめとした幕府の人々が"江戸城無血開城"のために奔走した幕末・大政奉還の物語を描く『幕末編』の2部構成となっている。そして音楽を担当するのは、前作も手掛けたKOHTA YAMAMOTO。本作でもドラマチックかつエネルギーに満ちた音楽で物語に彩りを添える。
